Database: Federal deficiency reports for Steward Health Care’s hospitals

In the past five years, Steward hospitals accrued more than 300 deficiency findings from federal hospital regulators.

The Globe analyzed more than 35,000 unique deficiency statements issued between June 2019 and June 2024 to nearly 3,400 hospitals across the US. These statements are based on surveys by federal investigators involving site visits, field observations, document reviews, interviews with staff and patients, and more.

The deficiencies found in Steward hospitals vary in seriousness and risk of harm to patients. Condition-level deficiencies represent a greater risk of harm or injury to patients than standard deficiencies. The term “immediate jeopardy” is reserved for the most serious and egregious failures — ones that put patients at immediate risk of injury or death.
